Sending big hugs to the two people who sent me DOES’s weekly certification form and the email address. Try as I might, I could not find this information on DOES’s website. That in itself is very frustrating because DOES is telling people to be sure to file their weekly certifications yet DOES’s outdated website is currently not accepting weekly certifications for PUA recipients or those who have met their benefits end year. Attempts to file weekly certifications over the phone are also not working for a lot of people. PUA recipients with their benefit year ending mid-March are having to wait for DOES to update the system with the new stimulus programs and extensions.
I’ve been told that you can fill out the FORM and email it to Fact.finding@dc.gov and DOES will receive it.
1:41PM UPDATE: Another kind soul has sent in a PUA Weekly Certification form with the Covid-19 questions! See below. I’m including both forms in this post.
APRIL 16 UPDATE: Someone sent me a message and told me that DOES is slow to process these paper forms. That said, if you can’t certify online or via the phone and DOES requires you certify weekly, this may be your only option for the time being.
