Job Description: Case Manager
Skyland Workforce Center, a nonprofit collaborative, engages individuals, business, government, and community-based organizations to provide a continuum of holistic, high- quality workforce development programs in one location, leading to work-ready employee candidates, job placement, economic self-sufficiency, and improved quality of life. On-site partners include Byte Back, Jubilee Jobs, Samaritan Ministry of Greater Washington, Southeast Ministry, STRIVE-DC, and Thrive DC.
As a pilot site for The WorkPlace DC (www.workplacedc.org), the Center serves as a hub for a range of services and opportunities for people working toward self-sufficiency through employment. Each person comes through our doors with unique experiences, circumstances, skills, and barriers. The Case Manager will meet them where they are and facilitate access to Center programs, including any wraparound supports they need to participate and to find and retain employment.
The Case Manager is employed by Building Bridges Across the River and reports to the Director of Workforce Initiatives, Skyland Workforce Center. The position oversees collaboration among partner organizations’ on-site caseworkers but does not have any direct reports.
Primary Responsibilities

Refine intake and orientation systems to facilitate low- to no-barrier access to
appropriate programs and services.

Directly support those individuals whose barriers require the greatest case management
expertise or experience to address

Provide mentoring, feedback, and advice to frontline caseworkers

Work with partner case management staff to develop processes for employment plans,
follow-through with current program participants, and follow-up with those who graduate, find employment, or drop out of programs.

Nurture deep collaboration among partners; coordinate information sharing among on-
site case managers as well as other partner staff, including accurate, timely, and
standardized use of the HIRE-DC shared database.

Communicate intake, orientation, and case management procedures to administrative
staff.

Train new case managers on Center policies and protocols.

Use program data and feedback from staff and community members to identify and
address service gaps.

Submit regular reports on case management participation, referrals, barriers identified
and addressed, placements, and other program outcomes.

Education and Experience
Required:
BSW with 5+ years or MSW with 3+ years of experience in the field
Proficiency with Microsoft Office Suite Experience using database systems
Preferred:
Working knowledge of Salesforce
Experience in a role that included coordinating or supervising others
