POSITION OVERVIEW: The Perinatal Care Coordinator (PCC) aids in addressing psychosocial and emotional concerns of clients of the Mother’s Rising program, by monitoring, and organizing individualized care plans, community resources, and other partnerships to meet identified needs; shares resources on external social service agencies to meet needs of patients for housing, legal, domestic violence, and substance abuse; an identify services to address social determinants of health based on outcomes and patient feedback. The PCC manages a team of Perinatal Community Health Workers (PCHWs), Care Specialists (CS), and Community Birth Workers (CBWs) to develop strategies to help each client's family system towards stability. The PCC must be highly organized, empathetic, compassionate, non-judgmental, and eager to help address client issues.
