Dear Community Leader:

United Medical Center  is requesting your assistance as we develop a Community Health Needs Assessment for the Hospital. We are seeking input from people who represent the broad interests of the community and those with special knowledge of or expertise in public health. You were selected because of your leadership position in the communities UMC serves. 

We are requesting your participation at one of three meetings, scheduled on Tuesday, April 9th from 6:00 to 7:30 p.m., Wednesday April 10th from 12:00 noon to 1:30 p.m., or Wednesday April 10th at 6:00 p.m. to 7:30 p.m.  Each of these group meetings will be conducted in the Board Room, 2nd floor of United Medical Center. 

At each meeting, we will provide a brief study which outlines key demographic and health care information pertinent to UMC and the communities we serve. In a discussion session that follows we are requesting your counsel to define our community's key health care needs and to help prioritize these needs.
